FOR NON-UNIVERSITY REQUESTS ONLY: The donation for the use of Chapel of the Sacred Heart is $1,750.00 (this includes a donation of $1,250.00 and a refundable $500.00 security/cleaning deposit). The donation for the use of Mary, Leavey Chapel is $250.00.

AGREEMENT AND SIGNATURE (to be signed by Contact Person) I have read, understand and agree to the Chapel Regulations (as stated above). I have also shared the Chapel Regulations with my vendors and guests, and I understand that I am responsible for any violation of the afore mentioned regulations by myself, party, vendors and or guests
